Booking Terms and Conditions for Skin and Medical Limited Welcome to Skin and Bones. To ensure a smooth and efficient experience, please review our booking terms and conditions carefully. By making a booking, you agree to the following terms: Booking and Deposit A non-refundable deposit of 50% of the total appointment cost is required at the time of booking. This deposit secures your appointment and will be deducted from the total cost of your treatment. Cancellation Policy Cancellations can be made up to 14 days (2 weeks) before your scheduled appointment without incurring additional charges. If you cancel within this period, your 50% deposit will be refunded to you. Cancellations made less than 14 days before the appointment will result in the forfeiture of the full deposit. Rearrangement Policy You may request to rearrange your appointment up to 48 hours before the scheduled time without losing your deposit. Any requests to rearrange made less than 48 hours before the appointment will result in the forfeiture of the deposit. Rearranged appointments must be rescheduled within 3 months from the original appointment date, subject to availability. Late Arrivals Please arrive on time for your appointment. If you are more than 15 minutes late, we may not be able to accommodate your appointment, and the deposit may be forfeited. No-Shows Failure to attend your appointment without prior notice will result in the forfeiture of your deposit. You will need to make a new booking and pay a new deposit for any future appointments. Payment The remaining balance of your treatment cost is payable on the day of your appointment. We accept debit/credit cards, and other payment methods as specified by the clinic. Medical Information Please provide accurate and complete medical history and information as required. Failure to disclose relevant medical information may result in the cancellation of your appointment and forfeiture of the deposit. Changes to Terms Skin and Bones Medical Limited reserves the right to amend these terms and conditions at any time. Any changes will be posted on our website and will apply to future bookings. Prescripton Terms & Conditions For Skin and Bones Medical Limited At Skin and Bones, we are committed to providing our patients with safe, efficient, and effective medical care. This prescription policy outlines our procedures and guidelines for issuing both paper and electronic prescriptions. 1. Prescription Types Paper Prescriptions: Issued during in-person consultations or upon special request. Electronic Prescriptions: Sent directly to your preferred pharmacy or via secure email. 2. Prescription Eligibility Prescriptions will only be issued following a consultation with one of our licensed medical practitioners. Repeat prescriptions may be provided for ongoing treatments as deemed appropriate by the clinician. 3. Controlled Substances Skin and Bones does not issue prescriptions for controlled drugs under any circumstances. 4. Requesting a Prescription During Consultation: Prescriptions are typically provided during your appointment. 5. Prescription Collection and Delivery Paper Prescriptions: Can be collected from the clinic reception during business hours. Photo ID may be required. Electronic Prescriptions: Will be sent to you via secure email. Please ensure we have your correct contact details on file. 6. Repeat Prescriptions Prescriptions for repeat medications can be issued as a one off issue. For this we will need to have a consultation and you will need to provide proof of regular prescriptions. 7. Prescription Fees Standard consultation fees apply. We do not charge extra for prescriptions. 8. Prescription Reviews Patients on long-term medication may be required to attend regular reviews to ensure the continued appropriateness and effectiveness of the prescribed treatment. 9. Prescription Accuracy and Safety Patients are responsible for providing accurate and up-to-date medical history and information. Any concerns about medication side effects or interactions should be reported immediately to your prescribing clinician. 10. Lost or Stolen Prescriptions If a paper prescription is lost or stolen, please notify us immediately. A replacement can be issued at the clinician’s discretion, and an administrative fee may apply. 11. Policy Updates Skin and Bones Medical Limited reserves the right to update this policy as necessary. Any changes will be communicated via our website and in-clinic notices.
